  1. If the Holocaust survivor is new to The Blue Card, please submit the Referral application with supporting documentation and cover letter.
  2. If the Holocaust survivor is a recent/current Blue Card recipient, please submit the Referral application with supporting documentation and cover letter.
  3. If the Holocaust survivor is applying for our Telephone Emergency Response System program (TERS), please submit The Blue Card TERS application with supporting documentation and a cover letter.
